'Your Personal Health Record' is a new initiative & part of a growing worldwide trend to encourage people to take an active interest in their own health. This book provides the individual with the means to document and carry with them their medical details. Although many medical records are computerized these days, these records are not accessible by hospitals, pharmacies or other doctors. They are not portable and medical details including medications are often reliant solely on the individual's memory.
